Alphaeon Credit
An Alphaeon Credit Card, offered by Comenity Capital Bank, is a simple way to pay for procedures, 
treatments, products, and services provided by dentists, dermatologists, ophthalmologists, and plastic surgeons. You can pre-qualify to find out if you are likely to be approved and for how much with no impact to your credit score. If you pre-qualify, simply select continue to complete the full application or ask your doctor’s office for help. When you’re ready, simply tell your doctor you want to use your Alphaeon Credit Card to pay.
As long as you spend $250 or more, you will be offered various promotional financing plans to make paying for your purchase easier. Alphaeon Credit Card offers credit lines up to $25,000 based upon credit quality. You must pre-qualify before submitting your full application in order to be approved. Once your application is submitted and you accept it, there will be a hard credit check. We recommend 6 month plans only. If you pay off the entire amount within those 6 months, there is will be no interest.
Payments are made monthly. The minimum payment required per month may not be enough to pay
off your entire balance within 6 months. Should you not pay off your balance within 6 months, you will
have to pay interest.

CosmetAssure
We do not take insurance. CosmetAssure is a supplemental insurance for cosmetic procedures that provides coverage in the event of complications either during or after your procedure. It is NOT optional, all patients undergoing surgery must have CosmetAssure. This is factored into your quote.
We will upload your information to them and provide you with a copy of your policy. Please see the brochure attached at the end of this packet for more detailed information on CosmetAssure.
